Payments and third-party services
Intake forms may be processed through Tally and payments may be processed through Stripe. Those providers have their own privacy and security practices. Do not submit information you are not comfortable sharing through the online form.

Information is shared only as needed to operate the service, comply with the law, process payments, and support patient communication. Use of this website alone does not create a physician-patient relationship.
